The form was to be filled with the sick or wounded soldier's basic information (identification, diagnosis, treatment) and then tied to his uniform with the string. The tag booklet was carried by every medic. This is the specific version intended for gas casualties. Each tag is complete with original string and comes with the original pencil, which is most of the time missing, that is attached with a string. There is 21 tags and 4 have been used/torn out. In very good to excellent condition.
